At Blessed Beginnings your time is valuable. It is as important to me as it is to you, that you get seen at your designated time. That being said, it is important that you show up on time for your appointment. This allows for you to have your time and gives the next person the time they deserve. Since everyone’s time is important, I have policies and fees regarding missed or canceled appointments to keep everyone accountable.

• New Patients
  

As a new patient, I like to get to know you and discuss with you your goals and treatment expectations. It is important that you fill out all your paperwork prior to your appointment. Arrive early to your appointment, so that we can start on time and review any paperwork or insurance issues. Also, please bring all your medication bottles with you from every doctor that you see. We will go over your past medical and psychiatric history, current symptoms, diagnosis, and treatment options. Not always is medication an option, it can also be a referral to therapy, spiritual healing, life coach referral, and or healthy lifestyle and living recommendations.

• Patient Forms and Policies
  

As a new patient, I will send you, via email, a link to fill out new patient forms. These forms will ask for personal information, like address, social security number, insurance, along with medical and psychiatric history. All information online is HIPPA compliant. You will initial and sign our policies stating that you understand our medication management and appointment adherence and fees.
